HIPAA

Notice of Privacy Practices

Effective date: October 1, 2026

This notice explains how we may use and disclose your health information, how to access it, and how to raise a privacy concern. It applies to Pillars Health Group and affiliate locations, including our mental health services and substance use disorder (SUD) records protected by 42 CFR Part 2.

Please read it carefully.

Privacy contact: Privacy Officer – Compliance@pillarshg.com. You may request a paper or electronic copy and discuss this notice with our privacy contact.

Your rights

  • Access and copies. Ask to inspect or obtain an electronic or paper copy of records we maintain about you. We generally provide a copy or summary within 30 days and may charge a reasonable cost-based fee.
  • Corrections. Ask us to amend information you believe is incorrect or incomplete. If we deny a request, we explain why in writing, generally within 60 days.
  • Confidential contact. Ask us to contact you in a specific way or at a different address. We honor reasonable requests.
  • Limits on sharing. Ask us to restrict uses or disclosures for treatment, payment or operations. We may decline, except that we generally must honor a request not to disclose to a health plan an item or service you paid for in full out of pocket, unless law requires disclosure.
  • Accounting. Ask for an accounting of disclosures for the prior six years, subject to applicable exceptions. One accounting in a 12-month period is free. Part 2 rights to certain electronic record disclosures and intermediary recipient lists apply when their regulatory compliance dates take effect.
  • Notice and representative. Obtain a paper copy of this notice, even if you chose electronic delivery. A legally authorized personal representative may exercise your rights after we verify their authority.
  • Complaints. Contact our privacy contact or the U.S. Department of Health and Human Services Office for Civil Rights, 200 Independence Avenue SW, Washington, DC 20201; 1-877-696-6775; https://www.hhs.gov/hipaa/filing-a-complaint/. You may also file a Part 2 complaint with the Secretary of HHS. We will not retaliate.

How we use and disclose information

For mental health information that is not protected by Part 2, HIPAA generally permits use and disclosure for treatment, payment and health care operations without written authorization. For Part 2 records, we generally obtain your written consent before using or disclosing identifying information for those purposes; one consent may cover future treatment, payment and operations. You may revoke consent in writing, except to the extent we already relied on it. Recipients covered by HIPAA may redisclose Part 2 records received under a valid treatment, payment and operations consent as HIPAA permits, subject to the legal proceedings’ protection below.

  • Treatment: coordinate care with treating professionals. Payment: bill you or your health plan. Operations: manage quality, staffing, audits and services. Part 2 information is shared for these purposes only with consent or another applicable Part 2 permission.
  • Your choices: subject to applicable law, you may share directly with family or others involved in your care and in disaster relief. We obtain written authorization for most marketing, sales of information and most uses or disclosures of psychotherapy notes. Part 2 consent rules also apply when the information is a Part 2 record.
  • Other permitted or required uses: as applicable, public health and safety, reporting abuse or neglect, research, health oversight and audits, legal compliance, certain disclosures concerning deceased persons, workers’ compensation, and specific government functions. Part 2 exceptions are narrower and may allow disclosures for medical emergencies, internal communications, qualified service organizations, research, audits, child abuse reports, crimes on premises or against staff, and certain court ordered disclosures. Each use or disclosure must meet the conditions of the law that permits it.

Special protections for SUD records

Part 2 records cannot be used or disclosed in civil, criminal, administrative or legislative proceedings against you without your written consent or a Part 2 compliant court order accompanied by a subpoena or similar legal mandate. An ordinary subpoena alone is insufficient. If we request such an order, the procedures required by Part 2 apply. We will not use your Part 2 records for fundraising unless we first give you clear notice and an opportunity to opt out. If we send other fundraising communications, you may tell us to stop.

If state law or another applicable law provides stronger privacy protection, we follow that law.

Our responsibilities and changes

We protect the privacy and security of your information, follow the terms of the notice in effect, provide a copy, and notify you of a breach when required. We obtain written permission when law requires it. We may revise this notice and apply revised terms to information already held. An updated notice will be available at our facility, on request, and on our website. We will post the current notice in a clear and prominent location.
