Individualized Discharge Planning

No two recovery journeys are the same, which is why discharge planning is highly individualized and developed in close collaboration with each client, their therapist, and the broader treatment team. Every aftercare plan is tailored according to the client’s primary diagnosis and clinical progress, ongoing symptom management needs, family and social support systems, work, school, and life responsibilities, as well as identified risk factors and relapse history. This comprehensive and personalized approach ensures that discharge recommendations are realistic, clinically appropriate, and thoughtfully aligned with the client’s goals, responsibilities, and long-term recovery needs.

Step-Down Care Coordination

Transitioning out of a structured treatment environment can feel overwhelming without proper support. Outstanding clinicians and phenomenal mental health care. I was hesitant to join but am beyond happy and eternally grateful for every group and one-on-one session I experienced. A very well rounded program that has created such a safe and comfortable space to build connections with others in recovery and to develop skills and healthy ways to navigate anxiety, stress, triggers, and unhealthy urges. I wish I could give more than 5 stars. A highly recommended program.

I’m still currently attending Pioneer Behavioral Health part-time three days a week, so I’m writing this while I’m still in the program. The whole team has been so supportive, kind and genuinely invested in helping me. I’ve already been able to see real changes in my life and in the way I handle things, and I feel more positive than I have in a long time about my ability to actually make lasting changes. I’m really grateful I found this program and for everyone who has been part of my journey so far. If I could rate Pillars & their care team 10 stars, I absolutely would! I cant even begin to describe what a blessing they have been for me, in SO many ways! I'd never done group therapy before this, so I didnt know what to expect. I told myself I'd put in the effort to go for a few days, but, once I got there & started attending groups, I was warmly welcomed by everyone & they quickly turned into a second family to me! I spent a little over 90 days here & during that time, I gained a lot of skills & relationships that I will forever cherish! Lasting impressions were made by Max, Hannah, Erica, Casey, & Amy.
