Dialectical behavior therapy (DBT) is a therapeutic modality that was originally designed to treat individuals with borderline personality disorder (BPD). However, it’s been adapted and improved to treat a wide range of mental health conditions including depression, post-traumatic stress disorder (PTSD), eating disorders, and anxiety disorders.

DBT combines cognitive-behavioral therapy (CBT) techniques with mindfulness practices such as acceptance and present-moment awareness. DBT is particularly effective for individuals who experience intense emotions and struggle with impulse control.

How We Use DBT in Tewksbury & South Portland

At Pillars, we use DBT in both our Half-Day & Full-Day Outpatient Treatment Programs. Both programs offer individual and group DBT sessions.

In individual DBT sessions, one of our expert therapists will work one-on-one with each client to explore their personal challenges, improve motivation, and reduce harmful behaviors. DBT helps the client stay committed to their treatment goals, as a key feature of the therapy is the therapist’s role as a coach and ally. Our therapists guide clients through intense emotions and gently guide them towards acceptance and change.

In group DBT, clients will meet in a supportive environment where participants can share their experiences, while getting support and learning from like-minded peers. This group setting fosters a sense of community, reduces feelings of isolation, and encourages accountability. Sessions can often focus on skill-building, particularly mindfulness, distress tolerance, emotional self-regulation, and interpersonal effectiveness.

Together, the individual and group DBT formats combine a personalized approach with peer support. Clients have the opportunity to manage emotions, improve their relationships, and reduce harmful behaviors.

Is DBT Right for You?

Deciding if DBT is an appropriate course of therapy depends on each client’s emotional challenges, behavioral patterns and treatment goals. DBT may be a strong fit if the client often feels overwhelmed by emotions, struggles with self-destructive behaviors, or finds it hard to maintain relationships.

DBT is especially effective for those who feel like they’re stuck in traditional one-on-one weekly or monthly therapy and not making progress. DBT offers structure, and skill-based tools to improve an individual’s quality of life.

DBT may be a good fit for those who:

  • …struggle with intense emotions that feel hard to manage
  • …engage in self-harming behaviors or suicidal thoughts
  • …have difficulty maintaining healthy relationships
  • …experience frequent mood swings or emotional outbursts
  • …feel overwhelmed in stressful situations and lack coping tools
  • …have tried other therapies but still feel stuck or unmotivated
  • …want a practical, skills-focused approach to mental health
  • …are dealing with borderline personality disorder, PTSD, or chronic anxiety
  • …crave more structure and support in the therapy process
  • …value mindfulness and want to improve present-moment awareness

I started at Pillar Health during a really difficult time in my life. From day one, the team treated me like a person, not a case number. The intake was thorough but never judgmental, and the group sessions actually gave me tools I could use right away. What stands out most is how the staff follows through and I can’t say enough about how caring they were about my situation. That kind of care is rare. If you’re struggling with mental health, substance abuse or just need someone in your corner, go to Pillar Health. They genuinely want you to get better.

Outstanding clinicians and phenomenal mental health care. I was hesitant to join but am beyond happy and eternally grateful for every group and one-on-one session I experienced. A very well rounded program that has created such a safe and comfortable space to build connections with others in recovery and to develop skills and healthy ways to navigate anxiety, stress, triggers, and unhealthy urges. I wish I could give more than 5 stars. A highly recommended program.

Pioneer behavioral health and the people there have seriously saved my life and changed it for the better. I cannot imagine what I would do without that program today I’ve learned so much in such a short amount of time I couldn’t be more grateful for the knowledge in the community that I’ve gained truly such a wonderful place.

I’m still currently attending Pioneer Behavioral Health part-time three days a week, so I’m writing this while I’m still in the program. The whole team has been so supportive, kind and genuinely invested in helping me. I’ve already been able to see real changes in my life and in the way I handle things, and I feel more positive than I have in a long time about my ability to actually make lasting changes. I’m really grateful I found this program and for everyone who has been part of my journey so far. I was so nervous at first to participate in a group setting, but this experience has been life-changing. ❤️

My experience at Pioneer has been incredibly helpful and life-changing. When I first started, I was struggling in ways that made it difficult to manage my emotions, relationships, and everyday life. The program gave me a safe and supportive environment where I could be honest about what I was going through without feeling judged. The staff genuinely cares about my progress and helped me learn valuable tools that I can continue using after completing the program. Through therapy, group sessions, and learning different coping skills, I gained a better understanding of myself and learned healthier ways to cope in difficult situations.

If I could rate Pillars & their care team 10 stars, I absolutely would! I cant even begin to describe what a blessing they have been for me, in SO many ways! I'd never done group therapy before this, so I didnt know what to expect. I told myself I'd put in the effort to go for a few days, but, once I got there & started attending groups, I was warmly welcomed by everyone & they quickly turned into a second family to me! I spent a little over 90 days here & during that time, I gained a lot of skills & relationships that I will forever cherish! Lasting impressions were made by Max, Hannah, Erica, Casey, & Amy. Thank you all, from the bottom of my heart! SO MUCH LOVE!♡
